Please fill out separate forms for each employee. Note that they must arrive with valid government-issued photo ID and for mask fit tests they must be clean shaven. (Checkboxes for hearing, lung, mask fit, and other assessments at bottom of list.)

Respiratory fit testing

Note employees must be clean shaven for this test. They should also have a back-up mask type to test, in case their first choice isn't a fit.

What date and time works best for this employee?

Our hours for testing at our Edmonton facility (Safety Coordination Services) are:

  • Monday to Friday, between 7:30 a.m. and 4:00 p.m.
  • After hours testing from 4:00 to 8:00 p.m. is by appointment only (call to inquire and set up an appointment: 780-485-3585)

We also offer testing at our Leduc facility (Leduc Safety Services) on Wednesdays between 9:30 a.m. and 4:00 p.m. by appointment only (call to inquire and set up an appointment: 780-485-3585)​
